Death in a Hospital
The following information is from the Shrewsbury and Telford Hospital NHS Trust (SATH) website:
The experienced Bereavement Care team at the Trust provides a caring and compassionate service, offering support and reassurance, information and guidance.
A member of the hospital medical staff, who has been looking after your loved one at the time of death, will issue a Medical Certificate of Cause of Death (death certificate).
If this cannot be issued straight away, you will need to contact the Bereavement Office after 11 am on the next working day to arrange an appointment to come in and obtain this.
The Bereavement Office will normally be able to provide you with the certificate within 1-2 days after your relative/loved one has died. In some cases, the doctor may need to speak to the Coroner’s Office first, and when this happens a short delay may occur before the Bereavement Office can issue the certificate. We will of course, keep you informed of this. If the death has been reported to the Coroner’s Office you will be advised of what will happen next.
